Case Coordination

Ongoing services will always include care coordination/case management with the aim to create successful outcomes in the home, school, and in the community.

For more than 20 years, SB&H has developed collaborative partnerships with school districts in Maricopa County. These partnerships provide an integrated model of behavioral health care and have produced measurable positive behavioral and cognitive outcomes for children attending school in these districts.

This model of integration places licensed therapists in school settings and allows parents, teachers, administrators and local communities the opportunity to address conditions negatively affecting a child’s development to improve behavioral and academic success for students with special needs.

The SACE will provide specific recommendations regarding the academic, independent living, social, vocational and counseling needs of students. We will provide a carefully planned transition, appropriate accommodations, and support students with ASD.
