Community-Based Programs
Our Justice-Involved and Community Support programs are designed to meet individuals where they are, offering compassionate, practical support that promotes stability, recovery, and safety. Through a range of community-based services, we work in partnership with local systems, organizations, and neighborhoods to reduce barriers to care and strengthen connections to essential resources. Together, these programs focus on supporting long-term well-being and creating pathways toward healthier, more stable lives.
Court Liaison
The City of Phoenix Justice-Involved Court Liaison program connects individuals involved in the court system with behavioral health services and community-based supports. Working in close coordination with the City of Phoenix courts and partners, the program helps reduce barriers to care, promote stability, and support successful engagement in treatment. By serving as a bridge between the justice and health systems, the Court Liaison helps individuals move toward improved outcomes and reduced recidivism.
In Home
A voluntary permanent supportive housing (PSH) service designed to provide necessary supports and services in your home and the community. Recovery-based approaches create opportunities to feel empowered and confident in maintaining the housing of your choice. The In-Home PSH team strives to offer support and flexibility when and how you need it the most.
SOR
Our teams are working to reduce overdose in the community by providing overdose prevention education, resources, and supplies. These services are available to the community with no enrollment required – individuals and organizations can simply contact us to get support.
