Services will be community-based taking place wherever is convenient for members and families including In-Home, Schools, and other providers in the continuum of care.

Three Community-based SACE site locations are available to serve members and families.

The SACE Level I Birth to Five site is located at Southwest Human Development, 2850 N. 24th Street in East Phoenix, AZ 85008, p: 602-468-3430 (Whole Child Care services).

Levels II, III, and IV will be served at Southwest Behavioral & Health Services' (SB&H) newly remodeled Metro Integrated Health Home Clinic, located at 11221 N. 28th Drive, Phoenix, AZ, 85029, p: 602-388-1700 (option 2), and SB&H's East Valley SACE located in at 1255 W. Baseline Road, #138, Mesa, AZ 85202, p: 602-388-1700 (option 3) (integrated care - Whole Child and Primary Care services).

The Case Manager is responsible for overseeing the entire clinical assessment and individual planning process throughout the course of treatment. If the recommended service is not available either within the hub of providers or within the community, SB&H will continue to communicate with community providers to meet the needs of the member and family.

All SACE Clinics are open Monday through Thursday until 7pm; Friday until 5pm.
