Sandra Bass, FNP-BC, Primary Care Provider

Sandra Bass was born and raised in Indiana, and has called the Phoenix area her home for over 20 years. After obtaining her Bachelor of Nursing degree, she became an Army nurse.  She attended Northern Arizona University and obtained her FNP. Sandra's outside interests include scuba diving, running, yoga, and gardening.

Briana Crowe, FNP, Primary Care Provider

Briana Crowe was raised in Arizona and currently calls the Mesa area home. She got her BSN from ASU in 2012, and her prior experience includes working in the acute care hospital setting as a cardiac and ICU nurse for 10 years. After deciding to go back to school to become an FNP, she graduated from Grand Canyon University in 2021. Her experience as a family nurse practitioner includes urgent care where she cared for a wide variety of patients and needs. She looks forward to continuing her journey with Southwest Behavioral & Health Services as a Primary Care Provider. She has been married to a wonderful man for the past five years and has three children ages 4 years, 2 years and 6 months old. Her children are the light of her life. She loves spending time with family, her husband, and kiddos; traveling; singing; and animals. She has one dog named Sophie who is 10 years old and a cat named Chessie who is 18 years old. She is a loving and involved aunt to 19 nieces and nephews, with another little niece or nephew on the way.

Amy Elsbury, FNP, Primary Care Provider

Born and raised in Kingman, Arizona, Amy Elsbury began her career in nursing at the age of 16 while working part time at a local nursing home. Amy found herself in nursing leadership roles for many years, and in 2018 made the decision to return to school to obtain her FNP. She graduated with her FNP in 2021, and brings with her over 20 years of nursing experience to people of all ages. Amy has a passion for helping others, and is excited to share this passion with her patients at Southwest Behavioral & Health Services as a Primary Care Provider. Amy's nursing experience includes long term care; medical, surgical, and critical care; and emergency services nursing. She has provided care to people of all ages with a multitude of physical and psychological conditions. Amy has been married for 17 years, and she and her husband share two children. The family enjoys spending time together during baseball tournaments, softball games, and Diamondback games. Amy is close with her parents, brother and sister-in-law, and two nephews.

Linda Lieber, FNP, Primary Care Provider

Linda Lieber became an RN in 2003 and worked in critical care on a cardiac unit for 15 years and two years as a hospice RN in home health care. She has been a nurse for over 20 years. While attending graduate school she worked at psychiatric inpatient and outpatient facilities. She always had a passion for helping others and becoming a nurse. Linda's father and best friend both were recipients of organ transplants. She moved to Arizona from Dallas in 1987. She loves the Cowboys! She has three adult children; two boys and one girl, all who live in the Phoenix metropolitan area. She has a 7-year-old granddaughter who she adores and enjoys spending time with. She enjoys gardening, traveling, hiking, kayaking, and going to the White Mountains. She has a tiny yorkie who is 18 years old and four turtles. Linda enjoys helping people and patients with their health and wellness as well as other needs they may have. She has provided care to patients with many different health problems and of all ages with many different physical and psychological conditions which include cardiac, respiratory, kidney, transplant patients, cancer, as well as chronic and acute care that she sees in the clinic. She enjoys meeting and helping all patients and brings that passion to the Southwest Family Medicine patients she serves.
